Report Card Templates
A report card is read by a parent in about a minute, and the part they remember is the remark rather than the marks. These four give the remark real space instead of a two-line box at the bottom.
Subjects, marks and grades are three columns you fill separately - keep the same number of lines in each and the rows align. Any subject you leave out simply is not there.
Parents read the remark before the marks. One specific strength, one thing to work on described as an action, and one line looking forward is the shape that works - writing report card comments has the phrasing, including what to avoid.

What goes on a report card?
The student and class, the term, a subject-by-subject record, an overall remark, and a signature. Attendance is often added, and belongs beside the term rather than in the subject table.
How should remarks be written?
Name one strength and one specific thing to work on, in plain language a parent can act on. "Needs to improve" tells a family nothing they can help with.
Can these be used for a homeschool record?
Yes. Replace the school name with your own heading; nothing in the layout requires an institution.
